Thank you for your interest in becoming a member of the Centacare Quality Care Advisory Committee. We value the opportunity to hear feedback from skilled and experienced professionals and advocates from the broader sector in which Centacare operates, including aged care and NDIS, who are willing to share your perspectives with us. Participation on this advisory committee is an opportunity to contribute to improved quality, risk and governance processes within Centacare.

Centacare is seeking applications from:

Healthcare and Social Services personnel directly involved in the delivery of aged care services within and outside of Centacare
Members of Consumer Advisory Bodies, Members of organised consumer advisory services and care recipient advocates who represent the interests of aged care recipients, disability & mental health care recipients and domestic violence care recipients.

I understand that membership of the Advisory Committee is voluntary and that I will only be reimbursed reasonable expenses incurred in travelling to the meeting *

I acknowledge the obligation to act with integrity, maintain confidentiality and adhere to Centacare’s Code of Conduct and conflict of interest processes *

I understand that to uphold best practice governance, Centacare will conduct background checks on prospective members and I consent to background screening *
